Our dedicated claims team will ensure your claim is dealt with as smoothly as possible. Due to the challenging times we are all experiencing our phone lines are extremely busy. You can let us know about a new claim using our Health (Illness) Claim Form or Bereaved Claim Form.

To allow us to deal promptly with your request, please ensure you include:

  • your plan or policy number
  • your full name
  • your address
  • how you would like us to reply and your contact details (e.g. telephone number or email address)
  • your signature, which provides with us with your consent to complete your instruction.

Once we have received your letter, we aim to respond to your request within 5 working days. Please note in order to process some requests, we may need to get in touch with you for additional information. To avoid any delays in being able to carry out your request, please include a daytime contact number or email address.

If you want to cancel your policy

To cancel your policy we will need confirmation of cancellation by all policy owners, including trustees, either in writing or over the phone.

Please download, complete and return the relevant form for the beneficiary or trustee change you would like to make.

If you hold a Scottish Widows Protect policy your financial adviser can help provide you with the appropriate trust form to complete.

  • Deed of Appointment and Removal of Trustees – This form gives the policy holder a simple way to remove a trustee from their duties by appointing another trustee in their place. The new trustee(s) will take legal ownership of the policy once appointed. They have authority to speak about the policy but cannot make changes. Trustees must be over the age of 18.
  • Deed of Appointment and Retirement of Trustees – This form should be used where up to two trustees wish to retire from their duties and the continuing trustee wishes to appoint up to two replacement trustees in their place. Please note this form should only be used if the policy holder is / are also a continuing trustee. The replacement trustee(s) will take legal ownership of the policy once appointed. They have authority to speak about the policy but cannot make changes. Trustees must be over the age of 18.
  • Deed of Appointment of Additional trustee form – This form gives the policyholder the option to add additional trustees onto their policy. The additional trustee will take legal ownership of the policy once appointed. They have authority to speak about the policy but cannot make changes. Trustees must be over the age of 18 .
  • Deed of Variation of Default Beneficiaries – This form gives the policyholder the option to amend their beneficiaries. Please note there must be two trustees on the policy to complete this form. If the policyholder is currently the only trustee they must fill in the deed of appointment of additional trustee form first. The beneficiary is the appointed person who will receive the proceeds from the policy at claim stage.
