What you need to apply
- Signed application form
- Signed lifetime allowance form
- Completed discharge forms
- Bank details for payment of pension commencement lump sum and income payments
- Income withdrawals details (if income being taken)
- Completed Confirmation of Verification of Identity
- Bank details. If you are supplying bank or building society account details online on behalf of your client, your client must be the account holder and must also be the only person required to authorise debits from the account
